ACGME defines neurology as diagnosis, treatment, management, prevention, and investigation of nervous-system disorders across the lifespan. Current requirements also incorporate inpatient and outpatient environments, teamwork, safety, and supervised progressive responsibility.

Liking the brain is not yet the same as liking Neurology. The work asks you to translate history and examination into localization, manage diagnostic uncertainty, communicate clearly during acute change, and care for people living with both sudden illness and chronic disability.

What you will experience

Your original VignetteRx work sample begins when a patient’s neurologic state changes after sign-out. You will reconstruct the timeline, establish baseline, identify localization clues, separate urgent from non-urgent information, prepare an Acute-Change SBAR, and identify when to escalate. You are testing whether localization, uncertainty, and team communication fit how you want to work—not being tested on independent management.
