The 2026 ACGME definition describes internists as adult-care specialists using comprehensive clinical problem solving, managing undifferentiated and complex illness and comorbidity, and coordinating care and multidisciplinary teams across clinical settings.

Internal Medicine is not “liking everything.” It is sustained adult problem solving: multiple conditions competing for attention, uncertainty that must be managed rather than ignored, careful prioritization, and coordination across teams and settings.

What you will experience

Your original VignetteRx work sample begins with one adult patient and several meaningful problems. You will create a focused one-liner, decide what matters first, identify what information would change your decision, map team and consultation needs, and consider safe transition planning. The point is not to prove that you know every answer. It is to notice how you think when complexity is real.
